Safety, Oversight and Quality (SOQ) Secure Submission Portal

This upload portal is for Residential Care and Assisted Living Facilities to securely submit the following forms to ODHS:

APD 0567 30-Day Involuntary Move-Out Notice (Download form | Instructions) — Submittable to Community-Based Care (CBC).
APD 0568 Less than 30-Day Involuntary Move-Out Notice (Download form | Instructions) — Submittable to the CBC.

Important: This portal only accepts form DHS 0567, form DHS 0568, and supporting documentation for those forms. Submissions of other forms will not be processed.
